SUO Membership Application

Application Process:

  1. All application materials must be received by the SUO-HNS office no later than July 31, 2020
  2. Application Timeline:

    • Application deadline - July 31st
    • Application review completed by the Membership Committee - September 22nd
    • Recommendation sent to Executive Council - September 29th
    • New member announcements made following the SUO/AADO/OPDO Combined Meeting - November 21st

Application Requirements:

  • Applicant should:
    • Voting Members
    1. Qualifications for Membership To be nominated for membership in the SUO-HNS an individual must hold a faculty appointment to an ACGME-accredited or Royal College of Physicians and Surgeons of Canada (Royal College)-accredited Otolaryngology – Head and Neck Surgery residency program, or hold a faculty position in a department/division of Otolaryngology – Head and Neck Surgery otolaryngology at an LCME-accredited medical school. in addition, they must satisfy one of the following additional criteria:
      • Have completed an appropriate residency training program and show promise of a successful career in academic Otolaryngology – Head and Neck surgery as a teacher and/or investigator, or
      • Hold a Ph.D. degree with a faculty appointment in an ACGME-accredited or Royal College Otolaryngology – Head and Neck training program or hold a Ph.D. faculty position in a department/division of Otolaryngology – Head and Neck Surgery at an LCME-accredited medical school, and show promise in academic otolaryngology.
    • International Members
    1. Academic faculty members in Otolaryngology – Head and Neck Surgery Departments outside the United States of America or Canada may request nomination for International Membership into the Society. To be nominated for international membership into SUO, an individual must hold a faculty appointment to an appropriately accredited Otolaryngology – Head and Neck Surgery residency program within their country or hold a faculty position in a department/division of Otolaryngology – Head and Neck Surgery otolaryngology, at an appropriately accredited medical school within their country. In addition, they must satisfy one of the following criteria:
      • have completed an appropriate residency training program and show promise of a successful career in academic Otolaryngology – Head and Neck surgery as a teacher and/or investigator within their country, or
      • Hold a Ph.D. degree with a faculty appointment in an appropriately accredited Otolaryngology – Head and Neck Surgery training program within their country or hold a Ph.D. faculty position in a department/division of Otolaryngology – Head and Neck Surgery at an appropriately accredited medical school within their country, and show promise in academic otolaryngology.
    2. International members may be nominated for international membership under the same reference letter criteria required for voting members. In cases where qualifications cannot be reasonably verified, a prospective International Member may not be granted International Member status. International members may vote in committee meetings in which they are appointed. . International members may serve on all committees except the Executive Committee, Nominating Committee. The Executive Council reserves the right to limit membership to other committees as indicated. International members will pay all applicable meeting registrations and annual dues for SUO membership.
    • Resident Members
      1. Residents in good standing in Otolaryngology training programs may be nominated for resident membership under the same reference letter criteria required for a faculty member. Such individuals must be:
      • Training within an ACGME-accredited or Royal College-accredited Otolaryngology – Head and Neck Surgery program and are required to state their year of graduation when applying for membership.
      • Show promise of a successful career in academic Otolaryngology – Head and Neck Surgery as a teacher and/or investigator, based on letters of recommendation
      1. Resident members may not vote. Resident members may serve on committees and may have meeting registration and annual meeting dues waived until completion of their stated year of graduation. Upon successful completion and graduation from an ACGME-accredited or Royal College-accredited Otolaryngology – Head and Neck Surgery residency training program, a resident member may convert their Resident Membership to full Voting Membership if they hold a faculty appointment to an ACGME-accredited or Royal College-accredited Otolaryngology – Head and Neck Surgery residency program or in a department/division of Otolaryngology – Head and Neck Surgery at a LCME-accredited medical school. A candidate who applies in this fashion must send a letter from their department/division chair demonstrating proof of such appointment to the Executive Administrator’s Office by August 15th of the year in which they will apply, without need for a new application form or additional letters of support. The Membership Committee will review their application in the usual fashion and forward to the Executive Council for potential inclusion into recommendation for election into the SUO as a Full Voting Member at the next annual meeting.



  • Required support materials
    1. Letter of proposal from applicant's department head.
    2. Letter of proposal from one current SUO-HNS member other than the applicant's department head.
    3. Copy of applicant's CV
    4. Copy of applicant's bibliography (if not provided in applicant's CV).
